Navigate to the Content Hub
Click Survey in the left sidebar
Hover over the survey you want and select Create Asset in the bottom right corner
You have several editing options available. In the right sidebar, you can adjust the image Background.
In the left sidebar, you can change the Font, adjust Text Alignment, toggle Attribution, and update the Background Type.
